* Are you willing and able to pick-up and dispose of the dog's waste at all times when out in public?

* Are you willing and able to attend regular training meetings in order to maintain active Puppy Raiser (1-2 meetings per month) or Puppy Sitter (1 meeting per quarter) status?

* Are you willing and able to complete regular written and/or in person evaluations for your assigned service dog in training?

Are you willing and able to provide regular transportation for your assigned service dog in training to vet appointments, training meetings, prison facilities, etc.?

* Do you agree to notify Retrieving Independence immediately at the first sign of injury or illness of your assigned service dog in training?

* Are you willing and able to provide transportation to the veterinarian designated by Retrieving Independence in case of an emergency?

* Do you agree to follow all feeding, grooming, and training protocols as detailed in the Puppy Raising Manual and explained by Retrieving Independence staff?

* Do you acknowledge that the dogs you may work with are owned by Retrieving Independence and that you are in no way entitled to ownership of the dog?

* Do you agree to return the dog to Retrieving Independence at any time upon request by Retrieving Independence for additional training or placement?

* Do you have friends or family members either on staff or incarcerated at a Tennessee prison?
